Your Performance Report Request Failed To Generate a Result

I am trying to figure out the "code" to get a result for my ADS-B.

I go to the FAA website https://adsbperformance.faa.gov/PAPRRequest.aspx

and it is of course unnecessarily complicated...I would think that a user could simply enter the tail number and up pops the history.

The airplane has a -261 transponder and the 250 GPS with a GTN-650.

I enter the following:

1090ES
Dynon SV_XPNDR-261
Dynon SV-GPS-250 and Garmin GTN-650

I know that the ADS-B is showing up because a non-government easy to use website called adsbexchange.com shows my airplane.

So with respect to the not easy to use government website am I selecting the correct dropdowns?

Which source are you using for the GPS input to the XPDR?
